Referrals
Referrals are accepted on any felony offense in which the offender is community control eligible, with the exception of sex offenses.
 
LMCCC does not accept referrals for Intervention in Lieu of Conviction (ILC). CBCF Programs are designed to divert offenders from prison. With ILC, there is no conviction; therefore these cases are not a diversion from prison.
 
Referrals are accepted from the Common Pleas Court Judge assigned the case, the County Prosecutor's Office, or the County Probation Department only.
 
 
 
When making a referral on a Sentencing case, please forward:
  • Referral Form(Click Here!)
  • Police Report (if PSI is not completed)
  • PSI (when completed)
  • Any evaluations/assessments completed on the offender
When making a referral on a Judicial Release case, please forward:
  • Referral Form(Click Here!)
  • PSI
  • Prison Behavior/Programming Reports
  • Your recommendation
If the offender had been under supervision prior to going to prison, please forward supervision notes.
When making a referral on a Probation Violation case, please forward:
  • Referral Form(Click Here!)
  • PSI
  • CD/Mental Health Assessment/Evaluation (if applicable)
  • Supervision notes
  • Your recommendation
