Contractor Licensing Requirements Shall Include-
-Contractor License Application: To be completed and approved by the Director or designee, to be kept on file at the Water and Wastewater Office. (Click Here For Contractor Application)
-Licensed Contractor’s Agreement: Signature to be kept on file at the Water and Wastewater office. (Click Here For Licensed Contractor's Agreement)
-General Liability Insurance: Proof of MINIMUM $300,000.00 (three hundred thousand dollars) of General Liability Insurance, with the Licking County Water and Wastewater Department named as the Certificate Holder.
-Emergency Repair Escrow: $500.00 Certified Check payable to Licking County Water and Wastewater, to be held in escrow in case an emergency repair is necessary due to work performed incorrectly.
-License Fee: $25.00 Cash, check or money order payable to the Licking County Water and Wastewater.
-Workers' Compensation Certificate: Copy of current certificate to be kept on file at the Water and Wastewater office.
